No axle washer required. Don't know what that other nut is for but the instructions, from memory, say leave it alone.
It holds the rear adjusting bolt in place but allows it to rotate. You don't need to touch it unless you are replacing the adjusting bolt if it is damaged by a careless mechanic!!
